Its perfectly possible that the design was good, and the parts published specs met the design specs, but the parts did not meet those specs. Like maybe if they were bought from a Chinese supplier? If you can screw up dog food, well - then nothing's safe. That's my theory anyway.

Low odds on an poor design.
Better, but still low odds on the builder using under-spec parts.
Fairly good odds the parts were not as strong as they were spec'd.
